Scoliosis
& Spinal Curvature*
Treatment.

Scoliosis is an abnormal lateral curvature of the spine measuring more than 10 degrees on Cobb angle measurement, often accompanied by vertebral rotation. It most commonly develops during the adolescent growth spurt (adolescent idiopathic scoliosis) but can also occur in young children, adults with degenerative spine changes, or as a result of neuromuscular conditions. While mild scoliosis may require only monitoring, moderate curves (25-45 degrees) benefit significantly from specialized physiotherapy and bracing, and severe curves may require surgical intervention. At Dr. Karolin Rockson PT - Best Neuro Ortho Physio Clinic in Vellore, our scoliosis program utilizes the evidence-based Schroth method — three-dimensional breathing and posture correction exercises specifically designed to de-rotate, elongate, and stabilize the scoliotic spine, slowing curve progression and improving posture, pain, and respiratory function.

Symptoms
  • Visible asymmetry of the shoulders, shoulder blades, hips, or waist when viewed from behind
  • One shoulder blade more prominent or higher than the other
  • A visible rib hump on one side when bending forward (Adam's forward bend test)
  • Back pain, particularly in adult scoliosis or larger curves
  • In severe cases, reduced lung capacity due to thoracic rib cage deformity
Causes
  • Idiopathic: no known cause in 80% of adolescent cases, thought to involve genetic factors
  • Congenital: vertebral malformation present at birth causing asymmetric growth
  • Neuromuscular: secondary to cerebral palsy, muscular dystrophy, or spinal cord injury causing asymmetric trunk muscle pull
  • Degenerative: age-related disc and joint degeneration causing asymmetric spinal collapse in adults
  • Functional: apparent curvature due to leg length discrepancy or muscle imbalance, not true structural scoliosis
Diagnosis
  • Adam's Forward Bend Test to identify rib or lumbar prominence indicating vertebral rotation
  • Standing full-spine X-ray (EOS or standard) for Cobb angle measurement
  • Risser sign assessment on X-ray to determine skeletal maturity and progression risk
  • Lung function testing (spirometry) for thoracic curves above 70 degrees

Clinical
Approaches

Schroth Method
3D postural correction using curve-specific breathing, auto-elongation, and de-rotation exercises to halt curve progression.
SEAS (Scientific Exercise Approach to Scoliosis)
Active self-correction integrated into functional activities to build automatic postural correction.
Respiratory Rehabilitation
Three-dimensional breathing exercises to expand compressed areas of the rib cage and improve lung capacity.
Core & Spinal Stability
Building deep spinal stabilizers to support the corrected posture throughout daily activities.

🏠 Home Exercises Guide

Strengthen your recovery. Perform these exercises strictly under pain-free limits:

📋 Execution Blueprint: Schroth Muscle Cylinder Exercise (Individualized)

  1. 1Stand in front of a mirror and identify your specific curve pattern (right thoracic, etc.).
  2. 2Adopt the Schroth corrected starting position: auto-elongate the spine upward from the pelvis.
  3. 3Breathe deeply into the concave (collapsed) side of the rib cage, expanding it outward.
  4. 4Use muscle tension on the convex (bulging) side to de-rotate and flatten the rib hump.
  5. 5Hold the corrected position for 30 seconds while maintaining rhythmic Schroth breathing. Repeat 5 times.

Common Questions

Moderate scoliosis (25-45°) can be significantly managed with bracing and Schroth physiotherapy. The goal is to prevent curve progression and improve function, not necessarily straighten the spine completely. Surgery is reserved for severe curves (>45-50°).
Bracing is typically recommended for curves between 25-40° in skeletally immature patients. Large, well-designed braces worn 18-23 hours per day have been shown in clinical trials to reduce the risk of curve progression requiring surgery by 60%.
The Schroth method is a specialized three-dimensional scoliosis physiotherapy approach using curve-specific breathing, elongation, and de-rotation exercises. It is the most evidence-based physiotherapy approach for adolescent idiopathic scoliosis.
Mild scoliosis often causes no pain. Adult degenerative scoliosis and larger curves cause back pain from asymmetric muscle loading and joint stress. Physiotherapy effectively reduces scoliosis-related pain through targeted strengthening and posture correction.
Swimming is excellent for general fitness in scoliosis patients. However, it does not correct or halt scoliosis progression on its own. Schroth exercises remain the evidence-based physiotherapy approach for curve-specific correction.
Yes, thoracic scoliosis above 70° can significantly reduce lung capacity by distorting the rib cage. Schroth's three-dimensional breathing exercises specifically target concave-side rib expansion to maximize respiratory function.
Yes, adult degenerative scoliosis can progress slowly (1-3° per year) even after skeletal maturity, particularly as disc degeneration progresses. Physiotherapy and core strengthening can slow this adult progression.
We track Cobb angle progression on standing full-spine X-rays every 6 months during skeletal growth, correlating with Risser staging. Progression of more than 5° over 6 months or a curve reaching 25° triggers escalation of treatment intensity.
