The Exercise Paradox in Fibromyalgia

Fibromyalgia patients typically avoid exercise because movement hurts. Yet exercise is the single most evidence-supported intervention for fibromyalgia — more effective than most medications. Breaking through the initial pain barrier is the key challenge.

Starting Safely: The 10-Minute Rule

Start with just 10 minutes of very low intensity exercise — the level at which pain does not increase during or after the session. Maintain this for 2 weeks before progressing. This builds the biological foundations for tolerable exercise.

Pacing Principles

Fibromyalgia requires activity pacing — spreading activity throughout the day, avoiding push-crash cycles (overdoing on good days causing flares). Regular, predictable activity is better than variable effort.

Aquatic Exercise

Warm water reduces pain sensitivity, buoyancy removes impact stress, and gentle resistance builds strength without flares. Start with water walking and progressive water exercises before transitioning to land-based exercise.
